You can definitely get a 515+ with Miledown. Both Miledown and I scored a 515+. Remember that anki is just a tool for content review. You supplementing your content review with books is a great way to go beyond Miledown's deck. Just make sure that you get adequate practice and review in. Using a premed deck takes time, I suggest you instead download miledowns document and go through it over and over while making anki cards for the content in UWorld. You can totally use a premade deck (boaras is significantly better than mile down.
