You have to craft equipment until an ancient appears. Ancients are not that rare, however, if you craft 20 or 30 pieces, you may get 3 or 4 ancients (that's my experience). I couldn't reforge them in kanai cube. You wouldn't want to anyway. It would be much cheaper materials-wise to just keep crafting new ones.
