There's a +/- button on the hotbar on the left side. Click the + to increase the hotbar size and - to decrease it. The top right corner of the hotbar has a padlcok icon. When it's locked your abilities are locked to the hotbar and can't be moved and when it's unlocked it can be customized. #1.

This can be toggled by pressing the lock or unlock icon that can be found on the right side of the toolbar and may be turned on or off at any time. Kill character and resurrect.
